Reust, MD, Center for Family Medicine Science, University of Missouri, Department of Family and Community Medicine, MA303 Health Sciences Center, Columbia, MO 65212 and Robert L. Blake, Jr, MD, Columbia, Arch Fam Med. 2000;9:282-283.

The research relates to what diagnostic workup is appropriate before making the diagnosis of colic?

The objectives of the research are as follows:

A MEDLINE search (1966-present) was performed using the key word “colic,” with limits to diagnosis, English language, and age group (newborn infants aged 0-1 month and infants aged 1-23 months). Seventeen articles were identified, of which 3 were specific for diagnosis. Two of these articles involved the diagnostic utility of acoustic analysis of crying, and 1 involved the use of a symptom diary. A similar strategy was used on MEDLINE (1996-present) to review infantile gastroesophageal reflux (GER).

How Is Colic Diagnosed? Many conditions can cause excessive crying in a baby, and colic is one of them. Colic is a term often used to describe persistent and excessive crying for no apparent reason during the first three months of life. Colic is diagnosed only after other medical conditions that cause excessive crying have been ruled out. The diagnosis is often confirmed after the fact, meaning once symptoms have improved. The History and Physical Exam Before diagnosing colic, your healthcare provider will begin by asking a number of questions. This will include questions about: The crying (when it happens, how long it lasts, and what it sounds like) What you do when your baby cries Your infant’s diet, stools, urination, and sleep patterns What and how you feed your baby Possible causes of the crying. Your healthcare provider will also examine your infant, looking for clues that might help explain the crying. Is It Colic or Something Else? There are many possible causes of prolonged crying in an infant. Your healthcare provider will consider these before making a diagnosis of colic. Some of these conditions include: Constipation Stomach flu (gastroenteritis) Middle ear infection (otitis media) Gastroesophageal reflux disease (see Infant GERD) Nasal congestion Thrush Overdosing on a drug (such as pseudoephedrine) Intussusception (whooping cough) Anal fissure Urinary tract infection (UTI) Shaken baby syndrome Meningitis Corneal abrasion (scratch to the cornea). There are also many non-medical reasons why babies cry, including hunger, pain, and being tired or overstimulated (see Understanding a Crying Baby).

Let’s face it, caring for a baby is hard work. Your little bundle of joy might cry more than you ever expected. Listening to a crying baby can be frustrating, especially when it never seems to end. As a parent or caregiver, you have limited energy, patience and tolerance. It’s common to feel overwhelmed, helpless and possibly even angry by the constant demands of the baby.

How to Calm a Crying Baby

Check physical needs first: Is the baby hungry? Thirsty? Need to be burped? Too hot or cold? Diaper dirty?

Check for signs of illness or fever. If you think the baby may be sick seek medical attention immediately.

Rock the baby, hold the baby close, or walk with the baby.

Stand up, hold the baby close, and repeatedly bend at your knees.

Sing or talk to the baby in a soothing voice.

Gently rub or stroke the baby’s back, chest, or tummy.

Offer a pacifier or try to distract the baby with a rattle or toy.

Swaddle the baby with a soft blanket.

Take the baby for a ride in a stroller or in a car seat in the car.

Turn on some music or noise like a vacuum cleaner or clothes dryer.

Try each of the above for a few minutes before trying something else, or try a few together. If nothing seems to work, it is OK to leave the baby in a safe place (like a crib or infant seat) and take time to calm down. Leave the room. Shut the door. Take a few deep breaths. Call a friend or family member.

Why does my baby cry so much?

The first cries of a newborn baby are often music to parents’ ears. However, as the weeks pass, this “music” can become grating and difficult to deal with. This is especially true when all attempts to stop the crying fails.

All babies cry a lot during the first few months of life. Crying is the way babies communicate. It can be challenging to distinguish newborn crying. However, as babies get older, parents can sometimes differentiate an “I’m hungry” cry from an “I’ve been over-stimulated” cry. Babies cry because they are hungry, uncomfortable, frustrated, tired or even lonely. Sometimes, cries can easily be answered with food or a diaper change.

It is important to respond to your baby’s cries. Contrary to “old wives’ tales,” young babies cannot be spoiled by being picked up when they cry. Being held is reassuring and comforting when a baby cannot express herself any other way.

Although babies cry in order to communicate, crying can continue for a long time with no apparent reason. Crying can be a mystery and it stops as quickly as it begins. Your baby is not mad at you and he is not trying to make you look like a bad parent. Even the most loving parent and caregiver can feel frustrated by a crying baby. Feeling frustrated does not make you a bad parent or caregiver. It is very normal to feel aggravated by crying.

Remember:

All babies cry and it is how they communicate.

Your baby may cry more each day until they are 2 to 3 months old.

Babies often cry more in the evenings

Crying can last 30-40 minutes and even longer. An infant may spend 2-3 hours a day crying.

Babies often cry intensely when they are not in pain, even though they may look like they are in pain.

Sometimes your baby may need to cry to relieve stress and it’s okay to let him cry.

Crying may come and go, and you won’t know why.

Crying may not stop no matter what you try.

Take comfort in knowing that the crying will eventually stop.

Your job as a parent is not to make your baby stop crying.

Speed is what makes TUMS different from other heartburn treatments, both over-the-counter products and prescription medicines. And that means feeling better faster when heartburn strikes. TUMS dissolves easily, travels quickly, and tackles heartburn at the source — neutralizing stomach acid right away. In contrast, some heartburn medications enter the bloodstream and must be processed by the body before they can begin to work. Several hours or days may pass before they take full effect to fight your heartburn. Acid reducers are heartburn medications that decrease the amount of acid produced by the stomach. H2 blockers (or H2 receptor antagonists) like Pepcid AC® lower the stomach’s acid production. Proton pump inhibitors (PPIs) like Prilosec OTC®, Prevacid® and Nexium® slow down the stomach cells that produce acid.

Jessica Simpson has a baby, or the story of a baby.

President Obama’s health care law includes a new entitlement about disabled. The Obama administration is that CLASS for Community Living Assistance Services and Supports must be eliminate. The administration is justifying the elimination due to the reason that it would wouldn’t be sustainable because of its cost. After much criticism from the leftists, the White House made it clear that the CLASS program would not be eliminated completely.

Premiums was being collected for years before any benefits would be paid and counting the money as savings and the program made the health care law looker cheaper than it really is. Republicans argue.

“A lot of the rest of the deficit reduction is smoke and mirrors, I believe, and I think it’s pretty easy to show,” Fox News contributor Bill Kristol said on “Fox News Sunday.” “I think the Republicans now need to say this is just the tip of the iceberg. We need to repeal Obamacare.”

The CLASS required to be self-sustaining, meaning it could not add to the deficit as language added by the Republican Senator.

Late Monday, White House officials seemed to hesitate a bit on halting the CLASS program. “We do not support repeal. Repealing the CLASS Act isn’t necessary or productive,” Assistant Press Secretary Nick Papas said.

Senate Republican leader Mitch McConnell said “it defies logic” for the White House to admit the program is unsustainable, but “demand it stay on the books.”

And the chief actuary of Medicare has warned the program would have to enroll 230 million people to be workable, or more than the entire American workforce.
